Considering Botox or filler injections? Dr Anita Soosay of The Aesthetics Centre Laser and Medical Aesthetics Clinic shares some important facts you should know. ®

Botulinum toxin type A (commonly known as botox) and fillers are injectable substances that can only be administered by a doctor. They are commonly used to tackle wrinkles, but there are also other applications. Dr Anita Soosay, medical director at The Aesthetics Centre Laser and Medical Aesthetics Clinic, tells us some, and explains the difference between the two.

Q: WHAT CAN BOTOX AND FILLERS DO, APART FROM TACKLING WRINKLES?

A: Both can be used to sharpen the features. If you have a round or square face, botox can be injected into your jawline to contour it, resulting in an oval-shaped face. Fillers can be injected into the nose bridge to make your nose appear higher, narrower or longer, or into the chin to sharpen it.

Q: IS USING BOTOX FOR FACE SHAPING SAFE? ARE THE INJECTIONS PAINFUL?

A: Yes, botox is a safe and quick method of face shaping when administered by an experienced doctor. You will look slimmer, yet still look like yourself! The injections feel like bee stings. However, the procedure is over very quickly and the pain is usually gone in a few minutes.

Q: I HAVE AN IMPORTANT EVENT COMING UP AND WOULD LIKE TO LOOK MY BEST. I’M CONSIDERING BOTOX AND FILLERS. HOW DO I DECIDE WHICH IS MORE SUITABLE FOR ME?

A: It is best to discuss this with your doctor, who will analyse your face and recommend the option more suited for you. Botox and fillers do different things: the former relaxes muscles, while the latter lifts. Sometimes, you get the best results when you go for both.

Q: WHAT DO I NEED TO KNOW ABOUT THESE TREATMENTS BEFOREHAND?

A: You can expect visible results 48 hours after botox, but for optimum effects, you’ll usually need to wait three weeks. Fillers, on the other hand, give immediate results, but there is typically some inflammation and mild swelling after the treatment. It is best to do both treatments at least four to six weeks before an important event. This gives enough time for the botox to take full effect and the swelling from the fillers to subside.

STEM-CELL THERAPY
What: Use your stem cells to generate new cells for brighter, firmer and younger-looking skin with improved elasticity and diminished fine lines and wrinkles.
How: There are two ways: Extract the cells from the bone marrow, or from abdominal and thigh fat, using syringes. The materials are processed and purified to separate the stem cells from the other stuff . Sometimes, the stem cells may be further cultured to increase their numbers. The concentrate is then injected into the targeted area.
In Korea, fat-derived stem cells are injected on the same day, while bone marrow-derived ones are injected after a four-week culture, says Dr Kim Byung Gun, a plastic surgeon and director of BK Plastic Surgery Hospital in Seoul.
Local doctors say: Stemcell therapy is not available in Singapore except in clinical trials approved by the Ministry of Health (MOH). It is not offered for aesthetic purposes.
• In its stead, Dr Low Chai Ling, medical director of The Sloane Clinic, recommends treatments using the fractional CO2 laser. Compared with other lasers, it reaches deeper into the skin where the collagen fibres are, to immediately tighten skin, and boost collagen production over the next few months. Its depth also enables more effective treatment of deep-set wrinkles and scars.
In South Korea, stem cells from bone marrow or fat, and blood platelets, are used as injectables for youthfullooking skin. 
PLATELET-RICH PLASMA (PRP) THERAPY
What: Inject your own blood platelets, which have proteins known as growth factors that contribute to wound healing. This supposedly enhances the skin’s natural repair process, thereby treating fine lines and wrinkles for a fresher, smoother complexion overall, with fewer blemishes such as freckles, scars and acne.
The jury is still out on the effectiveness of PRP therapy in dermatology, orthopaedics, sports medicine and other fields, but it hasn’t detracted from its popularity in Korea. “Koreans are more willing to try new things earlier than others,” says Dr Kim.
How: About 10ml of blood is taken from the patient and placed in a centrifuge, which separates the platelets from the rest of the blood. The platelet concentrate is then injected into the face. According to Dr Kim, who offers PRP therapy in his Seoul clinic, patients can expect to see improvements after several days, with results generally lasting between six and 12 months. Its effects are most often compared to those of fillers.
Local doctors say: Like stem-cell therapy, PRP therapy is only available here in clinical trials approved by the MOH. Otherwise, it’s mainly used in orthopaedic clinics to aid in the recovery of sprains and muscle tears.
• Dr Low suggests an alternative, Sculptra – an FDA-approved injectable made from poly-L-lactic acid, which encourages collagen production. Unlike hyaluronic acid fillers that create immediate volume to give the treated areas a supple appearance, Sculptra works gradually over a few months, reducing the severity of wrinkles and restoring facial contours as collagen levels increase.

Q: I’VE TRIED DIETING AND EXERCISING, BUT STILL CAN’T GET RID OF THE FAT ON MY ARMS AND MY DOUBLE CHIN. WHAT CAN I DO?

A: Try Exilis Elite, which utilises radio-frequency energy to heat and destroy fat cells. These are then removed naturally from the body. It also stimulates collagen production to tighten skin. During the 30- to 45-minute treatment, a gel will be applied, after which a handheld device will be glided over the skin; you may feel some warmth during this process. Four sessions every seven to 14 days are recommended. Expect some redness post-treatment. Results are usually visible after one session.

Q: IS THERE AN ALTERNATIVE TO LIPOSUCTION FOR MY TUMMY AND THIGHS?

You will experience a warm sensation during treatment, which lasts 45 to 60 minutes. The treatment area may feel sore after, as if you have exercised. Four to six weekly sessions are recommended and improvements are generally visible after four to six weeks.

Q: IS IT POSSIBLE TO TREAT CELLULITE?

A: Yes. I recommend BTL X-wave, which utilises acoustic radial soft waves – sound pulses that induce vibration – to prevent cellulite from returning or worsening. The feedback from those who’ve tried it is that it feels like a massage! It leaves no redness or marks, and can be done weekly. Improvements are usually seen after at least four sessions done one week apart.

INTRACEL (FRACTIONAL RADIOFREQUENCY MICRONEEDLING)
What: Korean company Jeisys invented Intracel, a machine that marries radio frequency (RF) and microneedling, where fine needles on a handheld device penetrate the skin to increase collagen production for firmer skin and to refine pores as well as reduce the appearance of wrinkles and superficial acne scars.
The machine’s latest version was given clearance by the US Food and Drug Administration (FDA) in July last year, and it’s reportedly one of the most advanced in the field.
How: The needles enter the skin, and RF pulses are released into the target area at precise depths. Topical anaesthetic is applied and left on for 30 minutes beforehand. Sessions typically last 30-60 minutes each. Some redness is expected, as well as possible slight swelling, which should subside within a day or two.
Local doctors say:
• Dr Gary Si, resident doctor at Gangnam Laser Clinic in Singapore (whose doctors are trained by Korean plastic surgeons), says Intracel addresses a variety of concerns. “It’s minimally invasive and effective in treating visible pores, wrinkles, and even deep and active scarring from untreated acne. It also helps promote collagen production for tighter, firmer skin with minimal downtime.”
• Dr Lim Kar Seng, consultant dermatologist at The Dermatology Practice, cautions, however: “Although Intracel works, the issue is the pain involved as well as the risk of infection – the micro-needles that breach the skin may potentially introduce a portal of entry for bacteria. The risk of infection is naturally higher than with non-invasive methods such as fractional CO2 or erbium lasers.”
PLEXR PLASMA EYE LIFT
What: It is the non-invasive alternative to blepharoplasty – the surgical fix for droopy eyelids that removes excess skin, muscle and fat. It utilises plasma, an ionised gas that manifests as tiny electric bolts, to stimulate the contraction of skin fibres in the upper or lower eyelid. Besides not having to cut the skin to remove fat, the treatment reportedly works on only the target areas (it doesn’t affect surrounding tissue or the skin’s deeper layers).
How: It’s performed under topical and/or local anaesthesia. “The tip of the Plexr device is applied in a specific order and manner to tighten excess skin in the area,” says Dr Si.
Local doctors say:
• Dr Lam Bee Lan, medical director at Ageless Medical Centre, describes it as “an innovative eye lift and eyelid treatment technology that’s great for correcting uneven folds around the double eyelid without surgery.”
• However, Dr Georgia Lee, medical director at TLC Lifestyle Practice Clinic, feels that the downtime can be relatively long. “Expect the treated area to swell. Scabs will start forming from the second day onwards, but fall off within a week.”

SCARED OF SPIDERS?

Spider veins, that is. Dr John Tan of The Vein Clinic & Surgery explains the causes of those red or purple clusters of thread-like veins and what can be done.

Love wearing short skirts, but not the unsightly spider veins (proper name: telangiectasia) on your legs? Or maybe you have them around your nose and are tired of covering them with makeup. Q: WHAT CAUSES THEM?

A: Spider veins – so called because of their fine, weblike appearance – can be hereditary. They can also be caused by increased blood pressure in the veins, which can result from prolonged standing or sitting, and hormonal changes such as pregnancy. Those on the face are often the result of sun exposure. Mostly, spider veins don’t hurt, although occasionally, they can cause aching, numbness and tingling.

Q: HOW CAN THEY BE TREATED?

A: Spider veins are not harmful to health and do not need to be treated. That said, if their appearance is troubling you or if they’re causing you pain, there are different approaches to tackling them. Many clinics offer laser and light therapies, but spider veins don’t always respond well to them. My preferred approach is to start with an ultrasound to exclude problems in the deeper veins, then treat the spider veins with sclerotherapy, a 30-minute procedure that involves the injection of a chemical directly into the affected veins, causing them to shrink, close and eventually disappear. Most people find it feels like a series of ant bites. I may combine this procedure with the use of a special laser for better results. Note, though, that spider veins may reappear, particularly if you become pregnant.

Q: BUT DON’T WE NEED VEINS FOR BLOOD CIRCULATION?

A: There’s no need to worry about that as the body has many other veins through which blood flows. The spider veins are already damaged, so eliminating them actually improves your circulation.

GOLKI THERAPY (BONE MASSAGE)
What: Created in 1979 by Lee Byung Chul, CEO of Yakson House beauty salon, the massage is said to help women (and men) improve their bone structure and blood circulation so as to achieve a smaller and more symmetrical face, brighter and bouncier skin, and improved wellness. It can be done on the face and body.
Yakson House has more than 100 outlets in Korea, Japan, China, the Philippines and – as of six months ago – in Singapore (#05-12A Wheelock Place, tel: 6734-8777).
“Fans of this holistic treatment include renowned Korean celebrities such as Gu Hara, Yu Ah In and Park Min Young. We also have regular clients from all over the world who visit Yakson whenever they are in Korea,” says Lee.
How: The areas that need “re-alignment” are deeply kneaded, using lifting, pressing motions, usually for 15 minutes. The treatment is performed only by native Korean therapists who have been trained at Yakson’s headquarters in Seoul.
We will tell you this: Golki Therapy hurts. Seriously hurts. To “sharpen and give the nose a higher bridge”, the therapist pressed her thumbs into the area between our nose and eye socket super hard, to the point where we felt she was going to crush our skull or remove an eyeball. We teared. A lot.
According to Lee, it hurts more for people who don’t exercise regularly or have a balanced diet as “they have a higher buildup of waste and toxins in their bodies”.
The pain supposedly lessens with subsequent massages because “there are less toxins to drain away”.
To see results, the salon says you need about 10 sessions (done weekly or fortnightly), and 20 sessions for the “re-alignment” to become permanent.
So far, according to Yakson House’s staff here, many customers, including Korean expats, have signed up for multiple sessions.
Local doctors say:
• Dr Georgia Lee says: “There is some medical basis for this phenomenon based on Wolff ’s law (developed by German surgeon Julius Wolff ), which states that bone tissue will adapt under load. Wolff published papers to support bone remodelling under pressure. Any immediate result is likely due to lymphatic drainage and oedema (swelling), though, as it will take months to see the effect of the pressureassisted bone remodelling.”
• Dr Low says: “From a scientific standpoint, it is difficult to say this can have any significant or longlasting impact. If there are any changes to your face shape, then we are also not sure if there may be any adverse effects over the long term, as there are no documented studies on this.”

A laser to remove pigmented tattoos... Peels for specific skin problems... Here, the latest doctors’ recommendations.

CUTERA ENLIGHTEN PICOSECOND TO REMOVE PIGMENTED TATTOOS

According to Dr Ram Nath, medical director at The Wellness Clinic, there are two pitfalls with traditional lasers. They can’t penetrate deep enough to remove coloured tattoos – pigments are deep set (compared to black tattoos), and green and blue are notoriously considered the hardest tones to erase.

They are also, um, not dark skinfriendly. “They can’t differentiate between the darkness of a strand of hair and the darkness of the skin,” says Dr Nath. Which means, dark skins end up being burnt and scarred more than fairer skins.

Enter Cutera Enlighten Picosecond, which is apparently the only laser that 1) penetrates deep enough to tackle pigmented tattoos, 2) differentiates between different pigments in a tattoo, and 3) is dark skin-friendly (it makes a distinction between the melanin in dark skin and the dark pigments in a tattoo).

“It delivers ultra-short pulses of energy in a trillionth of a second to disperse tattoo pigments into fragmented, minute particles, making them easier to eliminate from the body,” says Dr Nath.

The speed of the brief, ultra-short pulses is so fast that Dr Nath doesn’t even use numbing cream for the procedure. “The brain barely registers the pain or heat,” he says.

This cuts treatment time in half. Only two to three treatments are required to significantly reduce the visibility of a tattoo the size of a credit card.

Dr Nath adds: “Any redness or inflammatory responses are transient and will subside in an hour.” – CK

Xeomin, the new botulinum toxin

Launched recently in Singapore, Xeomin is supposedly one up on “botoxes”. It claims to be free of the binding proteins that are a natural part of botulinum toxins, but is one of the key causes of allergies. This means the body is less likely to develop antibodies in the long run and render the treatment less effective. – CK

MULTI-PURPOSE PEELS THAT SERVE SPECIFIC PURPOSES

Skin solutions shouldn’t come in a one-size-fits-all configuration. Ultraformer Hifu III

Hifu (High-intensity Focused Ultrasound) is known as the only non-invasive medical technology that delivers precise, highly targeted ultrasound waves to heat collagen fibres to approximately 65 to 75 deg C to “wound” them, which triggers healing and collagen production for tightened and lifted skin. The downside: it has a reputation for being extremely painful, with patients sometimes even requesting sedation. Ultraformer Hifu III is a third-generation machine that reportedly addresses this problem: The claim is that instead of searing pain from the pulses, all you’ll feel is a mild ache around bony areas such as the jawline. Bonus: the machine comes with a larger handpiece for use on the body, which was previously unavailable. According to Dr Yingzhou Tan, medical director at Simplistiq Aesthetic Clinic, who offers the device, “its ultrasound waves can penetrate to deeper depths of up to 16mm to tighten skin and reduce fats.” From $1,400 per session. – CK
